Notice to patients

 Arriving for appointment:

Please arrive the clinic prior 10-20 minutes early according to your appointment schedule, and fill out the “ patient intake form” .  We may need your medical exam reports and medical records, if any medical condition we need to know with your present health conditions.

 Preparations for treatment

● Please, do not arrive clinic with empty stomach

● Do not arrive clinic for treatment when you were very tired

● If you were pregnancy, please let us know

● If you have any special physical condition, please let us know

● For any handicapped patients or patients requiring special care, please let us know

●Please wear loose clothing for your treatment

 Follow up visit

You may need the follow up treatments, such as twice a week visiting, or once a week visiting for several weeks, or once every month according to your health condition.

 Follow up Phone call

Before each time of your follow up clinic visiting, we may give you a call for confirm the clinic visiting.

 Cancellations

      For cancellation of an appointment, please call our clinic day before of your  appointment.
